How do I report a malpractice incident or claim?

Claims must be reported directly to our office—they cannot be made online or via email.

To make a claim, please call our account managers at 800-343-1368 during business hours, (Monday - Friday, 8:00 am - 4:00 pm, CT), and have the following information available:

  • Date of the incident
  • Patient information (name, age, gender)
  • Facility information (name and address)
